Duly Health and Care ACO to Participate in ACO Reach to Strengthen Coordinated Care Efforts for Medicare Beneficiaries

DOWN­ERS GROVE, Ill. — Duly Health and Care ACO (“Duly ACO”) has been select­ed by the Cen­ters for Medicare and Med­ic­aid Inno­va­tion (CMMI) as one of 132 par­tic­i­pants in the ACO Reach (Real­iz­ing Equi­ty, Access, and Com­mu­ni­ty Health) mod­el for 2023. The per­for­mance year for this pro­gram began on Jan­u­ary 1, 2023, and it will run through 2026. Duly ACO will take respon­si­bil­i­ty for the qual­i­ty and total cost of care for a defined pop­u­la­tion of eli­gi­ble senior patients. 

Duly has par­tic­i­pat­ed in an Account­able Care Orga­ni­za­tion pro­gram since 2014. Most recent­ly, Duly ACO gen­er­at­ed over $24 mil­lion in sav­ings in the 2021 per­for­mance year in the Medicare Shared Sav­ings Pro­gram (MSSP) mod­el. As an orga­ni­za­tion focused on pro­vid­ing high qual­i­ty, acces­si­ble care for all patients, the ACO Reach mod­el is a nat­ur­al pro­gres­sion of Duly Health and Care’s val­ue deliv­ered to patients through exper­tise in Val­ue-Based Care and an inte­grat­ed out­pa­tient care deliv­ery mod­el. The ACO mod­el exem­pli­fies the way in which Duly’s physi­cians and care providers deliv­er excep­tion­al care in each inter­ac­tion with patients.

At Duly, patients are able to engage in their health and care with the sup­port of our providers and care teams through dig­i­tal tools intend­ed to make care nav­i­ga­tion eas­i­er. Duly offers over 50 med­ical spe­cial­ty and ancil­lary ser­vices to patients, and all providers use an inte­grat­ed elec­tron­ic med­ical record. At our ambu­la­to­ry surgery cen­ters, patients can under­go out­pa­tient surg­eries and receive oth­er ser­vices in a high-qual­i­ty and cost-effec­tive care set­ting. When patients receive their care with­in our out­pa­tient sys­tem, the Duly Health and Care app allows patients to view and man­age all of their care inter­ac­tions with our providers seamlessly.
