Duly Health and Care’s Charitable Fund invests in solutions that improve the health of our communities. Together with our nonprofit partners, we collaborate to address the social needs and risk factors that impact health beyond our clinic doors.
Our Impact
Founded in 2010, the Charitable Fund has granted over $4 million to support nonprofit partners with a focus on food security, affordable housing and support services, and access to healthcare and community wellness.

Last year, in 2020, the Charitable Fund granted nearly $670,000 in total. So far this year, the Charitable Fund has granted almost $500,000 to 50+ nonprofit partners (see list below).

Launched in 2019, the Associate Grant Program offers Duly Health and Care team members the opportunity to apply for small impact grants to support causes that are close to their hearts but may fall outside of the mission of the Charitable Fund.

Generous contributions from physicians, team members, community members and business partners are the lifeblood of the Charitable Fund. Duly Health and Care covers the administrative costs of running the Charitable Fund so that donations can go directly to nonprofits serving our community.

Our History and Structure
Founded in 2010, the Charitable Fund was established by Duly Health and Care physicians in partnership with DuPage Foundation to provide a coordinated approach of obtaining contributions and giving.
The Charitable Fund operates as a Donor Advised Fund (DAF) with DuPage Foundation, which is DuPage County’s community foundation and philanthropic leader. DuPage Foundation provides the Charitable Fund’s 501(c)(3) status and processes our donations. DuPage Foundation also provides the application portal and infrastructure for the Charitable Fund’s community grantmaking program.
The Charitable Fund is governed by an Advisory Board comprised of physicians, team members, and community members. The Advisory Board meets quarterly to review applications and provide grant recommendations.

Prospective grantees must receive an invitation to apply for funding. To determine eligibility, organizations may send basic information about their programs to the Charitable Fund at charitablefund@dupagemd.com.
