At Duly Health and Care, we want your child’s appointment to go as smoothly as possible. Completing required forms ahead of time helps us check your child in faster and avoid delays. Please complete forms before your visit either through MyChart or by downloading and printing the forms below.
Complete forms in MyChart
Follow the steps below to find and complete your child’s forms in MyChart:
- Log in to MyChart.
- Select your child’s profile in the top-right corner.
- Select the menu icon (three lines) in the top-left corner.
- Select Questionnaires from the menu.
- Select and complete the forms available for your child.
Please note: Forms are only available in MyChart after an appointment has been scheduled. Some forms may take time to appear, so check back if you do not see them right away.

Infants (Birth — 12 months)
TB Screening – 1 month old and 6 months old
The TB screening questionnaire is a tool used to assess an individual’s risk of tuberculosis infection or disease through questions about symptoms, exposure, medical history, and high-risk factors.

TB Screening – All children annually
The TB screening questionnaire is a tool used to assess an individual’s risk of tuberculosis infection or disease through questions about symptoms, exposure, medical history, and high-risk factors.

M‑CHAT – 18 months old & 24 months old
The M‑CHAT is a parent-report screening tool that consists of questions focused on a child’s behavior and development. It was developed to help healthcare providers identify early signs of certain developmental concerns.
