With your sur­gi­cal treat­ment com­plete, your care now tran­si­tions to our Duly Breast Can­cer Sur­vivor­ship Clin­ic. While you’ll con­tin­ue to see your med­ical and radi­a­tion 
oncol­o­gists as need­ed, your care team will now also include an Advanced Prac­tice Provider (APP) who will work close­ly with your surgeon.

Our goal is to help you achieve and main­tain a healthy lifestyle and to pro­vide sup­port and guid­ance for any issues that may arise. We will pro­vide you with a per­son­al­ized sur­vivor­ship care plan that includes:

Mon­i­tor­ing & Recur­rence Prevention

We’ll close­ly mon­i­tor you for any signs of recur­rence and estab­lish a fol­low-up sched­ule with reg­u­lar check-ups, mam­mo­grams, and oth­er imag­ing based on your indi­vid­ual needs.

Man­ag­ing Side Effects

Sup­port is avail­able for post-treat­ment chal­lenges like lym­phede­ma, with refer­rals to spe­cial­ists as needed.

Focus­ing on Wellness

Your over­all health remains a key focus, with access to resources and edu­ca­tion on nutri­tion, exer­cise, and smok­ing cessation.

Pre­ven­tive Screenings

We’ll help you stay up to date on impor­tant screen­ings, includ­ing colono­scopies, Pap tests, and skin checks.
