Concierge Med­i­cine at Duly Health and Care is a mem­ber­ship-based pri­ma­ry care option designed for patients who want enhanced access to their physi­cian and more time ded­i­cat­ed to their health.

With longer appoint­ments, same-day or next-day avail­abil­i­ty and quick access to your physi­cian for urgent con­cerns, Concierge Med­i­cine cre­ates more oppor­tu­ni­ties for thought­ful con­ver­sa­tions, per­son­al­ized guid­ance and proac­tive care.

You’ll also stay con­nect­ed to Duly’s full net­work of spe­cial­ists, imag­ing, lab­o­ra­to­ry and health­care ser­vices, so your care remains coor­di­nat­ed when you need more than pri­ma­ry care. 

Meet Our Concierge Med­i­cine Physicians

As a Concierge Med­i­cine mem­ber, you’ll have access to:

  • Same-Day or Next-Day Appoint­ments: Get prompt access to your physi­cian when a health con­cern comes up.
  • Longer Appoint­ments: Spend more time dis­cussing your health, ques­tions and goals, includ­ing dur­ing an extend­ed annu­al well­ness exam.
  • Direct Access to Your Care Team: Con­nect with the clin­ic direct­ly dur­ing office hours for quick call back when you need help or have a question.
  • 24⁄7 Access for Urgent Con­cerns: Reach your physi­cian direct­ly through secure, direct mes­sag­ing when an urgent health con­cern arises.
  • Con­nect­ed Duly Care: Con­tin­ue to access Duly spe­cial­ists, imag­ing, lab­o­ra­to­ry and oth­er health­care ser­vices as part of your broad­er care.
  • Expe­dit­ed Spe­cial­ty Appoint­ments: When spe­cial­ty care is need­ed, your concierge care team can help coor­di­nate an appoint­ment with a Duly spe­cial­ist with­in one week of refer­ral when clin­i­cal­ly appropriate. 

How Concierge Med­i­cine Works 

Annu­al Membership
A year­ly mem­ber­ship fee cov­ers the addi­tion­al ser­vices and access includ­ed in Concierge Medicine.
Insur­ance Still Applies
Con­tin­ue using your health insur­ance for cov­ered ser­vices such as labs, imag­ing, med­ica­tions and spe­cial­ty care.
Year-to-Year Flex­i­bil­i­ty
Choose each year whether you’d like to con­tin­ue your Concierge Med­i­cine membership.
Lim­it­ed Enrollment
Mem­ber­ship is lim­it­ed so par­tic­i­pat­ing physi­cians can main­tain a small­er patient pan­el and pro­vide the program’s added access.

What is Concierge Med­i­cine?

Concierge Med­i­cine is a mem­ber­ship-based approach to pri­ma­ry care that pro­vides enhanced access to your physi­cian, longer appoint­ments and more time for per­son­al­ized con­ver­sa­tions about your health.

What is includ­ed with a Concierge Med­i­cine mem­ber­ship?

Mem­ber­ship ben­e­fits include same-day or next-day appoint­ments, longer vis­its, quick clin­ic access dur­ing office hours, 24⁄7 physi­cian access for urgent con­cerns, con­tin­ued access to Duly ser­vices and sup­port coor­di­nat­ing expe­dit­ed spe­cial­ty appoint­ments when clin­i­cal­ly appropriate.

Do I still need health insur­ance?

Your exist­ing health insur­ance remains active. Insur­ance con­tin­ues to be used for cov­ered health­care ser­vices such as lab­o­ra­to­ry test­ing, imag­ing, med­ica­tions and spe­cial­ty visits.

Is there an addi­tion­al fee for Concierge Med­i­cine?

Yes. Concierge Med­i­cine has an annu­al mem­ber­ship fee that cov­ers the enhanced ser­vices pro­vid­ed through the pro­gram. Con­tact a par­tic­i­pat­ing physician’s office for cur­rent mem­ber­ship fee infor­ma­tion and details about how the pro­gram works with your insurance.

Can I still see Duly spe­cial­ists?

Yes. Concierge Med­i­cine mem­bers remain con­nect­ed to Duly’s net­work of spe­cial­ists, imag­ing, lab­o­ra­to­ry and oth­er health­care ser­vices. Your concierge care team can also help coor­di­nate spe­cial­ty referrals.

How quick­ly can I see a spe­cial­ist?

When clin­i­cal­ly appro­pri­ate, the pro­gram includes sup­port to help you see a Duly spe­cial­ist with­in one week of referral.

Is Concierge Med­i­cine avail­able with every Duly pri­ma­ry care physi­cian?

At launch, Concierge Med­i­cine is avail­able with Charles Schwartz, MD, in Elmhurst and Matthew Deck­er, MD, in Glen Ellyn. Mem­ber­ship is limited.

When does the pro­gram begin?

Concierge Med­i­cine with Dr. Schwartz and Dr. Deck­er begins Jan­u­ary 1, 2027. Enroll­ment is cur­rent­ly open.
