School & Annual Well-Child Physicals
The school year may look very different this Fall but keeping up with your child’s health and development will always be on your annual checklist. Whether learning continues from home or is back in the classroom, an annual school physical is the best way to check on your child’s health and wellbeing.
While the State of Illinois requires school physicals at four major milestones, a well-child visit is important every year. An annual physical with your child’s pediatrician or family medicine practitioner can give you helpful insight into your child’s growth and development over the last year, especially with as much change that has happened during these unprecedented times.
With same-day and next-day appointments and extended hours, we will get you in and out before the sun goes down. During this visit, you’ll have a comprehensive appointment with a physician you and your child trust and understands your family’s health history.

State of Illinois School & Sports Physical Requirements
The State of Illinois Certificate of Child Health Examination form must be submitted to your child’s school at the following grades:
- Preschool
- Kindergarten
- Sixth (6th) Grade
- Ninth (9th) Grade
- New students from out of state must complete a physical examination within one calendar year of the date of entry.
- Sports Physicals are required yearly for all athletes participating in school sponsored athletic programs. Sixth and ninth graders do not need a separate sports physical.
Your provider’s office will provide a completed State of Illinois Certificate of Child Health Examination form at your appointment.
