School & Annual Well-Child Physicals

The school year may look very dif­fer­ent this Fall but keep­ing up with your child’s health and devel­op­ment will always be on your annu­al check­list. Whether learn­ing con­tin­ues from home or is back in the class­room, an annu­al school phys­i­cal is the best way to check on your child’s health and wellbeing. 

While the State of Illi­nois requires school phys­i­cals at four major mile­stones, a well-child vis­it is impor­tant every year. An annu­al phys­i­cal with your child’s pedi­a­tri­cian or fam­i­ly med­i­cine prac­ti­tion­er can give you help­ful insight into your child’s growth and devel­op­ment over the last year, espe­cial­ly with as much change that has hap­pened dur­ing these unprece­dent­ed times.

With same-day and next-day appoint­ments and extend­ed hours, we will get you in and out before the sun goes down. Dur­ing this vis­it, you’ll have a com­pre­hen­sive appoint­ment with a physi­cian you and your child trust and under­stands your family’s health history.

State of Illi­nois School & Sports Phys­i­cal Requirements 

The State of Illi­nois Cer­tifi­cate of Child Health Exam­i­na­tion form must be sub­mit­ted to your child’s school at the fol­low­ing grades: 

  • Preschool
  • Kinder­garten
  • Sixth (6th) Grade
  • Ninth (9th) Grade
  • New stu­dents from out of state must com­plete a phys­i­cal exam­i­na­tion with­in one cal­en­dar year of the date of entry. 
  • Sports Phys­i­cals are required year­ly for all ath­letes par­tic­i­pat­ing in school spon­sored ath­let­ic pro­grams. Sixth and ninth graders do not need a sep­a­rate sports physical. 

Your provider’s office will pro­vide a com­plet­ed State of Illi­nois Cer­tifi­cate of Child Health Exam­i­na­tion form at your appointment.
